A lost, stolen or damaged licence is replaced by the NDLS for €35. You need a Garda Declaration form either way, whether you apply online or at a centre.
Step 1: complete the Garda Declaration form
The NDLS lists a completed Garda Declaration form as a requirement for every replacement, online or in person. Get the form and have it filled in before you start the application.
Step 2a: apply online
- Public Services Card and verified MyGovID
- The completed Garda Declaration form
- Proof of address from the last six months, if your address differs from the one on the lost licence and on your PSC record
- A medical report dated within one month, only if your medical details changed since your last application
- €35 by credit or debit card
Step 2b: apply at an NDLS centre
Book an NDLS appointment, then bring the Garda Declaration form, valid photo ID, proof of PPSN, proof of address if needed, a medical report if your details changed, and €35 (card, Google Pay, Apple Pay or Payzone).
Over 70?
The 70+ exemption covers first licences and renewals only. A replacement still costs €35.
If the licence was about to expire anyway
If you are within three months of the expiry date, renew instead of replacing. You pay the €65 renewal fee and get a new full-term licence rather than a copy of the old one. See Renew your licence.
Lost learner permit
The process is the same and also costs €35.
Need your driver number in the meantime?
Insurers usually only need your driver number (field 4d on the card). If you do not have it, you can request a free driver statement from the NDLS by email or post.
Where to go
Popular centres: Santry, Leopardstown, Citywest, Clarehall, Cork City, Galway, Limerick, Waterford, Kilkenny, Athlone, Sligo, Letterkenny, Drogheda, Tralee. All 34 centres.
